Student Update Forms

This week we sent home Student Update Forms for parents to review.  We cannot stress how important it is for us to have accurate information about your children!  Please ensure you take the time to review the information on the form and make any necessary changes directly on the form.  Once all of the information is accurate please sign both the front and back and return this form to the school.  Thanks for your cooperation in getting these forms back to us as soon as possible.
